RESERVATION REQUEST FORM

 

We welcome you back to the dining hall to experience our Chef’s Menu.

Very limited bookings are available from October 29th, 2021 and we’re giving you the first opportunity to book.

As a deposit is payable (cost of menu) to confirm any reservation with OTIS this is your early access to redeem your gift card issued due to COVID lock downs.

To use your gift card as your deposit / go towards your deposit please complete our ‘Reservation Request Form’ where we will manually apply your gift card to your reservation.

Once your reservation is confirmed you will receive notification via email.

If any details need to be clarified our Reservations Team will reach out to finalise your booking over the phone.  Please note that they are working remote and the call you receive will be from a mobile number (0473 132 171) which the restaurant phone is currently diverted to.

Please note that dietary requirements can be catered for however must be advised of prior to the evening of your reservation.

Please note that menus may change due to seasonality and availability of produce.  Dietary requirements can be catered for however must be advised at the time of making your reservation.

GOVERNMENT HEALTH ORDER: All staff & guests are required to adhere to current government health orders in relation to facemasks, QR check in, social distancing, vaccination status & our OTIS COVID Safe Plan as per Government requirements.

If bookings are made prior to vaccination status being reached & vaccination becomes unobtainable OTIS will refund your booking deposit in the form of a gift card in line with our refund policy below. For current health advise please visit ACT Health at www.health.act.gov.au

REFUNDS: Refunds are available in the form of a gift card where a minimum of 24 hours notice has been given. For added flexibility our gift cards have no expiry date.

DIETARIY REQUIREMENTS: Dietary requirements must be advised in advance. Please note these on your booking or contact us via phone prior to your reservation date to have these added.
